Is this a simile or a metaphor?

It's neither. I can see why you'd think so with the "...sister sat still as ..." which is a structure similar to similes. (Say that ten times!) A simile would be "..as still as a stone as she left..." or a metaphor would be "...she was stone as her sister left..."
